Description

The HOPE Assessment tool goes into effect on October 1, 2025. Every agency should have an implementation plan for HOPE that includes education. In this webinar we will review the new HOPE tool timing requirements and review question by question the HOPE assessment. We will discuss how to develop an implementation plan for the transition from HIS to HOPE.

Background

The HOPE Assessment Tool goes into effect on October 01, 2025. This will eliminate the current HIS. This new quality reporting tool includes changes in the frequency and timing of the assessment and includes numerous new assessment questions.

This is mandatory, and agencies that do not submit it will have a penalty of 4% in Medicare reimbursement.
